Free Pre-Diabetes Individual Appointments
Learn about the risk factors for diabetes and steps to manage pre-diabetes. A physician order is not required.
Diabetes Self-Management Classes
This program empowers you to control your own health. A physician’s order and pre-registration are required. Classes start in January and meet every other month, and continue for four weeks. Classes are offered on Tuesdays from 9 a.m. to 11:30 a.m. Call (219) 325-7623 for details.
Diabetes Support Group
The Diabetes Support Group provides a place to share ideas, experiences and problems related to diabetes for those with diabetes, as well as their support team. The group on the fourth Tuesday, 1 p.m. - 2 p.m. in January, March, May, July, September and November at Northwest Health - Starke in the first floor conference room. Registration is not required and the program is free.
Individual Appointments
A physician’s order and pre-registration are required. Appointments may be made with either a certified diabetes educator or registered dietitian, depending on your treatment method and needs: insulin, injectable, gestational diabetes, continuous glucose monitoring and nutritional education.
